New family checklist

1. 1Provide a current Immunisation History Statement from Medicare Australia - you can download this through your MyGov account.
2. 2Apply for the Child Care Subsidy via Centrelink in MyGov: complete the Activity Test and accept the CWA (Complying Written Agreement) for enrolment at Country Kidz Pre School.
3. 3Complete the enrolment form, the direct debit request form, the photographic and video consent form, and the 'All About Me' background form (this one helps us prepare for your child's first few weeks). All of these are included in your enrolment pack.
4. 4Download the Xplor app - we'll send an invitation to your email once your place is accepted.
5. 5Confirm your CWA within the Xplor app after installing it (step-by-step instructions are in the enrolment pack).
6. 6Arrange orientation visits with the Centre Director.
7. 7Pay the $100 deposit - the Director can give you the account details for the transfer.

Please label everything that belongs to your child.

• A full change of clothing: socks, underwear, top, jumper and pants or a dress, plus a coat or jacket for changeable weather.
• A comfort toy, if your child has one.
• A dummy if needed - with a cap, or stored in a small lidded container.
• Sturdy shoes to protect little feet during outside play.
• In wet weather: gumboots for outdoors and slippers to change into inside.
• A drink bottle or sipper cup for water.
• Bottles if required - prepared, named and capped ready to heat. Pop them in the Babies Room fridge when you arrive each day.
• Nappies? Don't worry - we provide those at the centre.
